SUMMARY: In
addition to providing overall direction and leadership for palliative
care, provides clinical care to patients and their families, is a
resource for health care providers, and participates in teaching
programs.
Work is performed with general independence and
decision-making responsibility in accordance with palliative care
clinical practice guidelines and scope of pra! ctice.
Guidelines include general policy procedures and pr! actice guidelines established by the department.
When
working as the attending ARNP on the Palliative Care Service is
responsible for conducting daily clinical rounds, and for providing
patient specific education as it relates to palliative care.
Responsible
providing palliative care clinical instruction to physicians, nurses,
social workers, chaplains, and others as requested.
Standards of Performance
Conducts daily patient rounds
Participates
in patient education and planning/reviewing lab tests, x-rays, and
interventions as determined in collaboration with the primary medical
team
Provides information to patients and their families
regarding diagnosis, expected course, prognosis, and care planning as it
relates to palliative care; ascertains patient and family understanding
Provides
assistance with establishing goals of care based on observations of the
patients as documented by self and or other! personnel
Participates in care conferences when appropriate
Provides episodic and hospital follow up consultations for patients and families as needed in the outpatient setting
Monitors
ongoing palliative care specific problems throughout the continuum of
care in the outpatient setting, including counseling and coordination of
resources
Provides care, consultation and education according
to professional practice standards, policies, procedures, clinical
pathways, protocols and guidelines, the Nurse Practice Act and Federal
and State regulations
Submits appropriate documentation in accordance with standards established for patient billing
Acquires, maintains, and applies current knowledge in the practice of palliative care
Contributes to the professional development of peers, colleagues and others, including members of the interdisciplinary team
Decisions
and actions on behalf of patients and families are determine! d in an
ethical manner and in collaboration with appropriate team m! embers
Participates and supports ongoing research activities
As
palliative care clinical faculty develops educational curriculum and
provides education via lectures and clinical expertise to all
departments of medicine and surgery
Considers factors related to safety, effectiveness and cost in planning and developing patient care
Develops policies and guidelines for the provision of palliative care across all settings
Participates
in grant writing and seeking alternative funding sources for palliative
care service and associated research projects
Evaluates quality
outcomes for palliative care service through the development and
participation in palliative care specific quality indicator activities
Represents Palliative Care Service in division meetings and committees as available

Standards of Practice
Demonstrates competency to assess physiological health status of the patient and psychosocial status of the! patient and family
Demonstrates
competency in assisting with implementation of and communication to
team members of plan of care based on health status data
Demonstrates competency in curriculum development and providing both didactic lecture and clinical instruction

competency in developing a comprehensive interdisciplinary plan of care
that includes interventions aimed at achieving expected outcomes
Seeks appropriate consults when necessary and follows up to ensure communication
Efficiently influences the implementation and the interventions identified in the plan of care
Evaluates the patient's progress toward attainment of outcomes and intervenes as appropriate
Demonstrate flexibility while striving as a specialized clinical practitioner
Palliative Care Specific Responsibilities
Is knowledgeable about plan of care for patients on service
Works effectively with interdisciplinary team
Demonstrates c! alm, efficient demeanor, is tactful and positive.
Available to staf! f for consultation and assistance
Serves as a resource for palliative care patients in the outpatient setting
Assists, resolves, or appropriately refers customer service issues
Offers expertise to hospital initiatives on protocol development
Ensures
facility safety for patient, family, staff, and reports issues to
appropriate persons consistently and in a timely fashion
Utilizes chain of command appropriately
Maintains list of patients receiving palliative care services or consultations
Provides an introduction to palliative care for medical center Patient Care Services orientation
Develops, facilitates, and coordinates palliative care teaching rounds monthly
Assures that plan of care and recommendations are communicated to primary service and to patients and their families
Consults with and supports social workers, therapists, care coordinators and charge nurses
Provides information direction to patients and their familie! s in relation to support groups
Develops curriculum and provides formal and informal palliative care education to physicians, residents, medical
students, acute care nursing staff, social workers, chaplains, and others as requested
In collaboration with other providers, participates in the after-hours on-call rotation for palliative care consultative service

Service Specific Responsibilities
Assumes
overall responsibility for the leadership and management of the
Palliative Care Service, including hiring and supervision of staff and
other attending providers when they are attending on the service
Develops new lines of business and revenue streams for Palliative Care Service
Assumes overall responsible for service budget, development, oversight, and adjustments based on revenue projections
Markets and provides informational updates to referring physicians in the community
Participates
in development of interdisciplinary! /programmatic quality improvement
plan by identifying areas for rev! iew, assisting with data collection,
and establishment and implementation of corrective actions
Provides education to patients, families, medical and nursing staff regarding outpatient hospice and palliative care services
Serves as primary provider and director of primary palliative care program.
Maintains current certifications in related field of specialty
Requirements

MINIMUM QUALIFICATIONS:
Graduate of an accredited
school of nursing with a Masters in Nursing as an Advanced Registered
Nurse Practitioner (ARNP) with an Adult or Family nurse practitioner
focus. Doctorate Preferred
Board certification as an advanced
practice hospice and palliative care nurse by the National Board for
Certification in Hospice and Palliative Nursing
Board certification by a nationally accredited agency as an Adult or Family Nurse Practitioner
Current WA State unencumbered license to practice as an Advanced Registered Nur! se Practitioner with prescriptive authority
Minimum of three years of professional nursing experience in palliative or hospice care.
Minimum of five years experience in a palliative care or hospice management and leadership role
Effective interpersonal skills, leadership skills, and clinical expertise in the care of patients with a life-limiting illness
