POSITION ESSENTIAL FUNCTIONS
Responsibilities:
- To provide social services and case management to patients in the ICU, Medical-Surgical, SWING-bed, ER, Infusion Center, and Surgery department settings.
- To work collaboratively with interdisciplinary and healthcare team members to coordinate care and services.
- To engage the patient/family in all aspects of case management and tailor services to the patient’s needs, preferences, and goals.
- To perform comprehensive psychosocial assessments and care planning for SWING-bed residents.
